C823 is a mostly sealed north-south route linking Lilydale Main Rd (B81) with Pipers River Secondary Rd (B83). It serves mainly agricultural land around the hamlets of Underwood and Karoola, although there are some vineyards located in the Karoola area. A 3.5km long unsealed section is located between Underwood and Karoola.

Prior to the construction of the Underwood Deviation in 1983, the Underwood Road section of C823 (from Underwood to Lilydale Main Road) formed part of Lilydale Main Road. The intersection of Karoola Rd, Underwood Rd & Brown Mountain Rd would have been the logical terminus of C823 had the deviation not been constructed. The deviation bypassed a narrow and gently winding section of road on the southern approach to Underwood, as well as eliminating the need for through traffic to descend into the valley and then climb out again.
